Description

This skill provides tools to assess readiness to change and tailor interventions accordingly. Use the brief readiness ruler for fast assessment and URICA access guidance for a more comprehensive profile.

Assessment Tools

URICA (University of Rhode Island Change Assessment) Access Guidance

Full assessment and documentation: → assets/urica.md

Interactive Administration (Optional)

Use this mode when the clinician says "start" or "administer" readiness screening.

  1. Readiness Ruler: Ask the 0-10 rating(s) (importance, confidence, readiness) and record scores.
  2. URICA: Use the official form and scoring guidance; have the patient complete it, then record 1-5 responses and compute stage scores.
  3. Summarize the stage profile, identify the dominant stage, and offer stage-matched guidance.

Clinical Workflow

  1. Select tool (brief vs comprehensive)
  2. Administer readiness ruler or URICA
  3. Score and identify dominant stage
  4. Match intervention to readiness level

Documentation

Use the templates in each asset file.

Overview

Provides tools to assess readiness to change and tailor behavioral interventions. Use the brief readiness ruler for fast triage and URICA access guidance for a more comprehensive readiness profile that informs treatment planning.

How This Skill Works

Clinicians select between a brief readiness ruler for a fast assessment or URICA for a detailed profile, administer the tool, and score results to identify the dominant stage. Readiness Ruler yields 0-10 scores for importance, confidence, and readiness in under a minute; URICA uses 32 items to generate stage scores for treatment planning. The resulting stage profile guides stage-matched interventions.

When to Use It

  • Quick triage during routine visits
  • Intake or initial assessment to gauge readiness
  • Planning behavioral interventions based on motivation
  • Complex treatment planning requiring a detailed readiness profile
  • Ongoing care with stage-based guidance to tailor interventions

Quick Start

  1. Step 1: Select tool (brief readiness ruler or URICA) based on case complexity.
  2. Step 2: Administer the chosen tool and record scores.
  3. Step 3: Score, identify the dominant stage, and apply stage-matched guidance.

Best Practices

  • Choose the tool that matches case complexity (brief ruler for quick checks, URICA for depth).
  • Administer and score consistently according to the asset templates.
  • Record scores and identify the dominant stage for each patient.
  • Use the stage-matching guidance to select appropriate interventions.
  • Document results using the templates in each asset file.

Example Use Cases

  • A clinician uses the Readiness Ruler for a 1-minute triage during a routine visit.
  • A patient with moderate readiness completes URICA to inform a detailed treatment plan.
  • Scores reveal a dominant contemplation stage, guiding a stage-matched brief intervention.
  • Results are documented using the provided templates in assets.
  • Stage-based guidance references are consulted to choose interventions.
